I Am Psychiatry and Sleep Despite popular belief most ADHD medications do not cause sleep problems In fact methylphenidate as well as longacting atomoxetine are both known to enhance sleep Adults who suffer from ADHD can still struggle to fall asleep They often toss and turn and wake up often Even when they do go to sleep their minds are still active Medicines A significant number of adults with ADHD suffer from sleep problems Although the scientific literature doesnt usually discuss these problems but they are reported to be common Patients parents and clinicians frequently report difficulty sleeping or staying asleep waking up at night nocturnal activity and sleepiness throughout the day Objective measures like actigraphy and polysomnography as well as subjective measures eg selfrated questionnaires and diaries have demonstrated statistically greater sleep problems in people with ADHD in comparison to people who are not The most commonly prescribed medication for ADHD is stimulant class medications These medications can increase focus and concentration however they may also affect sleeping in some patients It is important to rule out the primary sleep disorder like narcolepsy before taking these medications Spruyt and Gozal 2011 Stimulantclass medications can cause an abrupt increase in wakefulness and an increase in the efficiency of sleep The sleep disturbances could be caused by a decrease in the production of melatonin or directly affecting the nervous system In addition stimulants can induce a sedating effect paradoxical agitation in some individuals A randomized controlled trial of ramelteon an agent that stimulates the melatonin receptor and is believed to regulate circadian rhythms showed that insomnia improved for those with ADHD treated with this drug in comparison to placebo This was a tiny preliminary study with little power and a limited controls but it should be a catalyst for future studies to further explore the dosage timing and typing of the insomnia and levels of blood during the daytime of ramelteon and its counterparts Other drugs that have been used to treat insomnia in people with ADHD include clonidine trazodone and mirtazapine However they are not as researched and many doctors prefer not to use them in those with ADHD because of their potential for sedation to the next day If someone with ADHD has trouble sleeping or insomnia the first thing they should do is to practice good sleep hygiene and take an dose of their usual ADHD medication 45 minutes prior to going to bed In my experience this method is generally successful for a majority of people with ADHD and sleep issues If this doesnt work you can try a melatonin supplement Diet The kind of food a person eats can have an impact directly on their mood A diet that is rich in nutrients and balanced can help improve ADHD symptoms including sleep problems Foods should be low in processed fats and sugars and rich in nutrients such as fruits vegetables lean proteins whole grains and healthy fats including omega3 fats It is essential to cook at home to ensure that your meals are stocked with all the necessary ingredients for good health However many people suffering from ADHD struggle with planning and cooking their meals In these cases it is possible to rely on takeout or restaurant meals which are high in sugar sodium and artificial ingredients that may exacerbate ADHD symptoms for certain individuals People with ADHD often have trouble settling to sleep at the end of the night Sleep problems are one of the most frequent complaints of people with ADHD These sleep issues can cause a decrease in quality of life and an increase in depression anxiety and stress Sleep problems may be caused by a variety factors including signs of ADHD or other issues It is possible that certain medications used to treat ADHD such as methylphenidates could cause sleep issues in some individuals In addition those suffering from ADHD tend to have smaller pineal glands which could affect the way in which the body regulates its circadian rhythm To help people with ADHD to sleep better doctors usually recommend the use of a variety of treatment options This includes creating an established routine for sleeping and optimizing your sleep environment such a creating a dark and quiet bedroom and limiting the time you spend on your computer before going to bed Adults should also make sure they get enough sleep which typically is about 79 hours each night Exercise can help people with ADHD sleep better Before going to bed taking a few minutes of gentle stretching and deep breathing exercises can relax the mind and body This will result in more restful sleep Exercise Certain people with ADHD are unable to stick to a regular workout routine A personal trainer will provide structure motivation and accountability while incorporating an individuals preferred exercise into a fitness program Incorporating aerobic exercises into your daily routine could provide mental and physical benefits Mindbody exercise and strength training like yoga Tai Chi or breathing exercises can also be added to the routine of your day to take a holistic approach in managing ADHD symptoms Apart from helping people lose weight and build muscle mass exercise has other beneficial effects on the brain including increasing levels of norepinephrine as well as dopamine Norepinephrine and dopamine are neurotransmitters that enhance control and concentration Exercise increases the number of neurons that are growing and strengthens their connections This is especially evident in the cerebellum a region that contributes to fidgeting in certain people with ADHD Research has proven that judotraining for example can elevate dopamine and norepinephrine levels in the cerebellum more than just running on a treadmill Medications used to treat ADHD can have adverse side effects including decreased sleep This is why its important to discuss the persons medication with their physician or a health professional and to establish healthy sleeping habits Many people with ADHD struggle to fall asleep They might wake up often toss and turn or fall into what some call the sleep of the dead Their minds wander from one thought to another until they finally stop their daytoday activities and fall into a light sleep In my practice at least 80 percent my adult patients with sleep problems show this pattern These problems arent simply caused by ADHD and are actually an essential part of the disorder itself Adults suffering from ADHD might have trouble keeping an accurate internal clock which could be a factor in their difficulty falling asleep They also have trouble assessing the length of time theyve been in the bed In fact the process of completing a sleep study typically requires several nights of sleep logs and can be difficult for someone with ADHD to do accurately Sleep Environment Sleeping well is a challenge for everyone but it is particularly difficult for those who suffer from ADHD This is due to the inherent restlessness and racing thoughts that are associated with ADD make it difficult to fall asleep and relax Additionally many of the drugs used to treat ADHD can cause sleepdisrupting side effects such as insomnia Therefore it is essential that those with ADHD adhere to a healthy sleep routine and adhere to a regular routine They should also set up a calm relaxing bedroom In addition people with ADD often have sleep disorders that may affect their ability to enjoy adequate amounts of restful and restful sleep These include sleep apnea in which breathing stops in a sporadic manner throughout the night and anxiety depression and for women over age 40 menopausalrelated issues such as night sweats People suffering from ADHD should always have their sleep issues evaluated by a doctor to identify the root of the problem Research has revealed that people who suffer from ADHD experience poorer quality sleep than other people and are more likely to feel tired and unfocused during the day This is partially due to their restless sleep which may be exacerbated by their inability to control their internal clock However it is possible that the sleep problems are the result of medications that cause side effects or comorbid disorders People with ADD particularly those who are treated with stimulants often have difficulty falling asleep and waking up often at night In some studies the sleep onset delay and sleep efficiency of people taking stimulants were reported to be significantly lower than those taking placebos Some people have found that taking their ADHD medications prior to bedtime can improve sleep This should only be done under the guidance of a doctor since certain medications can trigger sleep disorders Adults with ADHD can benefit from other methods to promote sleep like progressive muscular relaxation in which you contract your muscles and then release them and slow deep breaths Its important not to make use of your bed for anything other than sleeping or sex
