Objective The aim of this study was to evaluate the results of the innervated digital artery perforator IDAP flap and to analyze the relationship between patient satisfaction and outcome measures Methods A total of 17 fingertips of 15 patients 14 men and one woman mean age 472 2662 years were included in this retrospective study Patients injured finger and defect type were recorded At the last followup the static twopoint discrimination s2PD test Semmes Weinstein monofilament SWM test and range of motion of the affected finger were analyzed We interviewed patients to determine hand dominance cold intolerance and their satisfaction with the result We performed correlation and logistic regression analyses between patient satisfaction and outcome measures Results The mean followup period was 138 718 months The mean range of motion was 77335 7080 degrees for the distal interphalangeal joints of affected fingers The mean s2PD was 64 310 mm and the SWM records ranged from 283 to 493 monofilament markings Cold intolerance was noted in seven fingers 41 Patient satisfaction was negatively correlated with cold intolerance and cold intolerance decreased as the followup period extended Conclusion IDAP flap satisfies both patient and surgeon with the only significant problem being cold intolerance regarding which patients must be informed Although cold intolerance is hard to treat fortunately it generally improves with time Level of evidence Level IV Therapeutic studyObjective The aim of this study was to analyze the results of the split anterior tibialis tendon transfer SPLATT to peroneus tertius PT for equinovarus foot deformity in children with cerebral palsy CP Methods The medical records of 25 ambulatory CP patients mean age 8732 years range 416 years with equinovarus foot 33 feet who underwent SPLATT to PT surgery between 2014 and 2016 were retrospectively reviewed A senior surgeon performed all the surgical procedures SPLATT was performed as part of a singleevent multilevel surgery for the lower limb and the concomitant procedures on the same extremity were recorded The patients who required any additional foot or ankle surgery that could affect the clinical outcome except heel cord lengthening were excluded from the study The Klings College Criteria were used to evaluate the procedural outcome of the foot position and gait and the associated complications were recorded Results The mean followup time was 2885 months range 2442 months The postoperative Kling scores were excellent for 27 feet of the patients who had a plantigrade foot without fixed or postural deformity in a regular shoe having no calluses good for 5 cases for those who walked with less than 5 varus valgus or equinus posture of the hind foot wearing regular shoes having no callosities and fair for 1 case for those who had recurrence of the deformity There was only one wound detachment which was treated with wound care and dressing None of the patients had overcorrection infection or bone fracture Conclusion The dynamic SPLATT to PT surgery for the management of the equinovarus foot deformities in the CP patients is a safe and less complicated surgical alternative with a good functional outcome It is a safe and effective treatment method for the management of equinovarus foot deformities in CP Level of evidence Level IV Therapeutic studyObjective The aim of this study was to evaluate clinical and functional outcomes in diabetic patients undergoing tibiocalcaneal arthrodesis using a retrograde nail Methods A total of 12 diabetic patients 8 men and 4 women mean age at intervention 568 years range 2776 years who underwent tibiocalcaneal arthrodesis by a retrograde nail were enrolled in this study The indication for surgery was massive talar osteonecrosis in four patients Charcot arthropathy in another four patients and various severe anklehindfoot derangements in four patients All surgeries were performed by the same surgeon All patients were evaluated by their American Orthopedic Foot and Ankle Score AOFAS score and radiographic followup was performed Results The mean followup time was 595 months range 27121 months Ten patients 833 healed and were able to walk with full weight bearing without crutches Among them nine patients 75 achieved union with solid bone healing The mean overall improvement in the AOFAS score was 725 preoperatively 40 points vs postoperatively 69 points p less then 0001 We observe a complication in 50 of our patients Minor complications included two cases of dehiscence of the surgical wound one case of soft tissue irritation owing to hardware protrusion and one cause of lymphedema Two patients had deep infection and underwent surgical removal of hardware debridement and antibiotic treatment one healed after the treatment but never recovered full weight bearing and the other one died from other complications These two deep infections occurred after 23 months of followup Conclusion Tibiocalcaneal arthrodesis using retrograde nails is a salvage technique extremely effective in ankle and hindfoot disorders in a diabetic patient This procedure allows good functional outcomes and pain relief When correctly indicated it is a safe procedure with good clinical outcomes and low risk of belowknee amputation Level of evidence Level IV Therapeutic studyObjective This study is an evaluation of the overall survival rate and factors affecting survival in patients with osteosarcoma chondrosarcoma or Ewings sarcoma This study aimed to determine the effect of factors related to the preoperative period patient tumor treatment and postoperative period on survival Methods A total of 114 patients 64 male and 50 female with osteosarcoma chondrosarcoma or Ewings sarcoma treated between 2005 and 2013 were included in this study All the patients received standard treatment and were followed up regularly In all 44 cases of conventional and telangiectatic osteosarcoma 30 cases of Ewings sarcoma and 40 cases of highgrade chondrosarcoma were identified using the Bone and Soft Tissue Tumor Registry Gender age tumor size and localization pathological fractures histopathological type grade surgical treatment adjuvant treatments relapse of the disease and postoperative complication data were obtained from followup forms The learning curve of inthe tumor Increase in the surgical experience is likely to have a positive effect on survival rates especially for patients with osteosarcoma The relapse of the disease is a poor prognostic factor for survival despite aggressive surgery and adjuvant therapies Level of evidence Level IV Therapeutic studyThis article devotes to solve the faulttolerant control problem based on interval type2 IT2 TakagiSugeno TS fuzzy stochastic switched uncertain timedelayed systems with signal quantization Stochastic switched systems can model a dynamic structure susceptible to abrupt faults making it more practically significant in power systems or economic systems The core design is an observerbased faulttolerant control scheme that can estimate incomplete measurable variables and eliminate the influence of fault dynamically well and enhancing the robust stability of the systems subject to quantization effects A novel method in seeking the upper bound solution of timevarying delay efficiently decreases conservativeness especially for the proposed timedelayed system The simulated analysis is specified to verify the availability and validity of the obtained design methodA model can be easily influenced by unseen factors in nonstationary environments and fail to fit dynamic data distribution In a classification scenario this is known as a concept drift For instance the shopping preference of customers may change after they move from one city to another Therefore a shopping website or application should alter recommendations based on its poorer predictions of such user patterns In this article we propose a novel approach called the multiscale drift detection test MDDT that efficiently localizes abrupt drift points when feature values fluctuate meaning that the current model needs immediate adaption MDDT is based on a resampling scheme and a paired student ttest It applies a detection procedure on two different scales Initially the detection is performed on a broad scale to check if recently gathered drift indicators remain stationary If a drift is claimed a narrow scale detection is performed to trace the refined change time This multiscale structure reduces the massive time of constantly checking and filters noises in drift indicators Experiments are performed to compare the proposed method with several algorithms via synthetic and realworld datasets The results indicate that it outperforms others when abrupt shift datasets are handled and achieves the highest recall score in localizing drift pointsIn this article we discuss challenges and strategies for evaluating natural language interfaces NLIs for data visualization Through an examination of prior studies and reflecting on own experiences in evaluating visualization NLIs we highlight benefits and considerations of three task framing strategies Jeopardystyle facts openended tasks and target replication tasks We hope the discussions in this article can guide future researchers working on visualization NLIs and help them avoid common challenges and pitfalls when evaluating these systems Finally to motivate future research we highlight topics that call for further investigation including development of new evaluation metrics and considering the type of natural language input spoken versus typed among othersSerologic studies are crucial for clarifying dynamics of the coronavirus disease pandemic Past work on serologic studies eg during influenza pandemics has made relevant contributions but specific conditions of the current situation require adaptation Although detection of antibodies to measure exposure immunity or both seems straightforward conceptually numerous challenges exist in terms of sample collection what the presence of antibodies actually means and appropriate analysis and interpretation to account for test accuracy and sampling biases Successful deployment of serologic studies depends on type and performance of serologic tests population studied use of adequate study designs and appropriate analysis and interpretation of data We highlight key questions that serologic studies can help answer at different times review strengths and limitations of different assay types and study designs and discuss methods for rapid sharing and analysis of serologic data to determine global transmission of severe acute respiratory syndrome coronavirus 2Wunderlich syndrome WS is a rare triad of flank pain flank mass and hypovolemic shock and is classically attributed to angiomyolipomata or neoplasms Treatment is guided by clinical severity conservative selective arterial embolization or nephrectomy We report an atypical case of a 69year old man with a preexisting 9 cm left renal tumor who developed WS secondary to anticoagulation and simple cyst rupture from his contralateral kidney complicated by abdominal compartment syndrome with hemodynamic instability despite inotropic support and robust resuscitation Early recognition and source control via radical nephrectomy were essential in securing a positive outcome
